If you’re a case manager, discharge planner, or part of the care team at a North Shore community, you know how much a resident or patient’s transition depends on the support waiting for them outside your walls. Active Companions provides that support: personal, rehab-informed companionship and in-home care for seniors across North Vancouver and West Vancouver.
Active Companions is led personally by Michelle Garceau, who brings over 10 years of experience working in rehabilitation with seniors and works closely with physiotherapists and care teams. Referrals are met by the same person who provides the care, not a call center, and not a rotating roster of unfamiliar staff.
How Active Companions can support your residents and patients:
Senior companionship: regular visits that reduce isolation and support day-to-day wellbeing
Movement & rehab support: guided, physiotherapy-informed exercise and mobility support between clinical appointments
Downsizing & life transition support: practical and emotional help moving into or between care settings
How a referral works:
Call or email with a brief overview of the resident or patient’s situation
A short conversation to understand their needs, routines, and any physiotherapy or care program already in place
A visit is scheduled, with updates shared back with you as appropriate
